2. Create a 4-8 second video in "portrait" mode, agree to our "Terms & Conditions" and submit your video and contact info via the form below.
 

• Create your video in "portrait" view/mode
   

• Focus on the visual only
  (no worries about scripting or background sound/noise)
   

• Make sure your video is clear/good quality
   

• Let your footage be inspired by your love and the music & lyrics of the song
   

• Keep it simple and real: Just be natural & use your phone camera
  (If you have previous footage from another time, please edit it down to 8 seconds)

Without expectation of compensation or other remuneration, now or in the future, I hereby give my consent to Firewoman Music DBA "Allowing Your Success & Beyond!, LLC", its affiliates, and agents, to use my image and likeness for potential placement in a music video for the song, "My Holiday", and in advertising/promotional, and social media posts directly related to the promotion of the video and song.
 

This consent includes, but is not limited to: (a) Permission to use any portion of the video I submit in all video-based platforms including YouTube, Vimeo, FirewomanMusic.com, social media platforms, or any other relevant entity (b) Permission to use my name and any and all information submitted by me (with the exception of my contact information, which will be kept private) (c) to use my image/likeness and information submitted by me (with the exception of my contact information, which will be kept private) in publications, in newspapers, magazines and other print media, on television, streaming sites, radio, and electronic media (including the Internet with social media), in theatrical media and/or any avenue that would be used to share and promote the video and song. This consent comes without compensation regardless of any amount of revenue the content may generate, is given in perpetuity, and does not require prior approval by me.

Translation:

You share your video clip for the joy of celebrating your love and getting to be featured in the music video for our song, "My Holiday", and understand this is not a paid gig.

You also are allowing us to credit you and include your names and how long you've been together in the video, social media, and any other places that would allow for the promotion of the song and video.
 

We will not share your email address/contact info with anyone for any purpose unless you give direct and express permission. We think sharing others' contact info to get spammed is a very crappy, UNloving thing to do!
 

We need your email so we can let you know you were chosen and/or when the video launches, but that's all.
